PRIMARY CARE SERVICES

New Patient Visit (45–60 minutes) – $165
Comprehensive evaluation, medical history, preventive screening review, treatment planning.

Extended New Patient Visit (60–75 minutes) – $199
For complex medical history, multiple chronic conditions, or expanded evaluation.

Follow-Up Visit (20–30 minutes) – $89
Lab review, medication refill, stable chronic condition management, minor acute concerns.

Extended Follow-Up Visit (30–45 minutes) – $140
Complex care management, medication adjustments, multi-condition follow-up.

MENTAL HEALTH SERVICES

Psychiatric Intake Evaluation (60 minutes) – $275
Comprehensive psychiatric assessment, diagnostic evaluation, risk assessment, treatment plan, and medication initiation (if indicated).

Extended Psychiatric Intake (75–90 minutes) – $325
For complex trauma history, co-occurring disorders, or extensive medication history.

Medication Management Follow-Up (20–30 minutes) – $135
Symptom review, medication monitoring, dose adjustments.

Extended Psychiatric Follow-Up (30–45 minutes) – $165
Complex symptom management or significant medication changes.

Reduced Self-Pay Rates (If Applicable)

Reduced-rate appointments may be available for qualifying uninsured patients. Approval must be granted prior to the appointment and may require completion of a financial hardship request form.

Payment Terms

• Payment is due in full at the time of service.

• We accept major credit/debit cards and HSA/FSA cards.

• A card may be kept on file for appointment scheduling purposes.

Failure to remit payment at time of service may result in rescheduling of the appointment.
